The following information is also required :-

 

• A full alcohol and drug assessment preferrably less than  3 months old.
• A treatment plan.

 

Authorisation from a DHB approved credentialed clinician. If a person is already in a treatment programme, the credentialed clinician who first authorised entry to that programme needs to be contacted to discuss the referral to The Trust.
